If you are currently enrolled at another institution, please have the registrar forward an additional official transcript at the end of the semester or term (final transcript). If you are a member of Phi Theta Kappa at your currently enrolled institution, please ask that your PTK membership is indicated on your official transcript.

  6. Feel free to provide any additional information about your schooling, interests, or family or work experiences that you feel is appropriate. This may be communicated in an essay on Screen 8, attached to the application agreement statement, or discussed in person during an on-campus interview.
